Policy Notification

To provide the best service possible,
Myo Wellness has implemented the following policies:

Online Booking

For your health and safety, you will be required to complete a thorough intake form prior to your first session and again once every 12 months. This form can be completed online in advance of your appointment. You will also receive a brief form to complete prior to each session to provide the reason for your visit. Your information will be kept confidential and will only be released by your consent or when required by law. 

Cancellations/No Shows

I respectfully ask that you provide a 24 hour notice for any schedule changes or cancellation requests. Please understand that when you cancel or miss your appointment without providing a 24 hour notice, I am often unable to fill that appointment time. As a sole practitioner, this greatly affects my ability to provide for my family. For this reason, you will be charged 100% of the service fee for each late cancelation or missed session. 

 

I understand that emergencies, illnesses, and inclement weather can prevent clients from providing timely notice. Late cancellation due to emergency, illness, or inclement weather will not result in a cancellation fee. If you have a fever, a known infection, or experienced vomiting or diarrhea within 24 hours prior to your appointment time, I request that you cancel your session until the illness has subsided.

Late Arrivals

I ask that you please aim to arrive 5-10 minutes prior to your appointment time to allow time to fill out any required forms and briefly discuss your treatment for that session. I understand that issues can arise that may cause you to be late for your appointment. Please call or text to notify me if this ever occurs so I can do my best to accommodate you. Appointment times are reserved for each client, so oftentimes I cannot exceed that reserved time without making the next client late. For this reason, arriving after your appointment time may result in loss of time from your massage so that your session ends at the scheduled time. Full service fees will be charged even when sessions are shortened due to late arrival.

Refusal of Service

I reserve the right to refuse service to any client for inappropriate behavior, illness or contagious conditions, unidentified skin conditions, or failure to follow current policies. If a session is ended early for any of these reasons, you will be responsible for the full service fee.
